To submit a monitor for repair, please complete the form below. Please note: the address you provide will be used as the return address. FORTEC Benelux B.V. is not responsible for damage that may occur during transport to our facility. Please make sure the monitor is securely packed before shipment. Once your request has been submitted, we will keep you updated on the status of your repair by email.
